CCS is a tax supported statewide program that helps children and young adults up to 21 years old who have certain medical conditions. CCS provides services such as arranging, directing, and in some situations, paying for medical care, occupational and physical therapy, equipment, and rehabilitation.<br> <br>Under the supervision of the Senior PHN, the PHN will act as the Public Health Nurse Case Manager to perform case management tasks that will assist the CCS client in the receipt of appropriate and timely access to community and specialized health care services. The PHN's duties will include the processing of authorization requests; care coordination of health care services with community health care providers; facilitation of physician and patient collaboration in supporting medically necessary treatment services via management education, support and advocacy. In addition, the Public Health Nurse will work with the Medical Therapy Unit (MTU) Team on mutual cases to enhance the medical and community coordination of care for the child.<br> <br>The current vacancy is a <strong>full-time, limited-</strong><strong>term </strong>position.
