Certified Nursing Assistant (CNA)
Life Services Alternatives, Inc. (LSA) is a California non-profit Corporation serving Individuals with Intellectual or Developmental Disabilities (IDD). Our mission is to provide personalized, dignified, and lifelong living experiences. LSA currently serves Individuals living in residential homes under license from the Department of Social Services. LSA operates in Santa Clara County, CA.
POSITION: Certified Nursing Assistant (CNA)
WORKING HOURS: 5-8 hour shifts - AM, PM, NOC shifts
REPORTS TO: Home Administrator
FSLA STATUS: Hourly/Non-Exempt
PAYSCALE: $24.50 per hour to $26.50 per hour
JOB SUMMARY:
The Certified Nursing Assistant (CNA) is directly accountable to the Home Administrator. The CNA provides direct support and teaching for the individuals living in the home. The primary responsibility of the CNA is to teach skills and behaviors and to provide care to maximize independence in activities of daily living.
PRIMARY RESPONSIBILITIES:
· Serve as a role-model to the men and women living in the home
- Assist Individuals served to increase their independence and to exercise their rights by teaching skills in various activities of daily living according to formal plans and as natural teaching opportunities arise
- Assist Individuals served to reduce fewer desirable behaviors and to learn appropriate replacement behaviors by implementing approved behavior management procedures
- Assist in maintaining the health, hygiene and grooming of Individuals served by teaching new skills
- Document care and assistance provided and behavior management and teaching techniques implemented by accurate and timely completion of data and Individuals’ notes
- Continually engage Individuals served in age-appropriate, functional activities and implementing community access activities, self-government and choice
- Participate in the IPP planning process by giving input on the progress and barriers regarding Individuals’ objectives
- Administer medications and treatments and maintain accurate data in the Medication Administration Record
- Notify to relevant parties if a significant incidence occurs
- Orient per diem staff to the home, ensuring implementation of daily schedules, ensuring documentation is maintained, ensuring medications for all Individuals are administered at appropriate times and maintain knowledge of the whereabouts of all the Individuals when out in the community
- Provide on-going transportation needs of the Individuals living in the homes
· Responsible in maintaining a current CNA license and provides the Home Administrator with the license renewal
· Additional responsibilities as assigned as circumstances warrant
QUALIFICATIONS:
1. Minimum age is 18.
2. Holds a current Direct Support Professional certification and current CPR and first aid certified preferred
3. Has a minimum of six (6) months experience in providing direct supervision and services to people with developmental disabilities preferred
4. Demonstrates caring, patience, good customer service skills, and ability to respond to Individuals we serve and co-workers in an empathetic manner
5. Maintains sufficient strength, agility, endurance and sensory ability to perform the duties contained in this job description
6. Holds a current CNA Certification
EDUCATION:
1. Holds a high school diploma or equivalent
2. Displays proficiency in written and oral English. Ability to communicate through verbal and written mediums
3. Attends Direct Support Professional training Part 1 in the first year and Part 2 in the second year. If no experience, will need 12 continuing education training units within 6 months of hiring
4. Holds a current CNA Certification
ADDITIONAL REQUIREMENTS:
· This position is a mandated reporter, and is required to observe all Individuals’ rights, report unusual occurrences as specified in Title 22, California Code of Regulations
· Hold a valid California driver’s license as it is an essential part of the position, current auto insurance, and a good driving record
· Undergo fingerprint and criminal record check, and provide results of health screen and TB test prior to hire date
· Be a citizen or alien authorized to work in the U.S. with legal documentation
WORKING ENVIRONMENT:
Typical Working Conditions: Primary work location is indoors, outdoors for leisure activities. Works varying hours and/or varying shifts, mandatory overtime may be required to assure the health and safety of our Individuals. Regular interaction with Individuals in providing direct services, including group guidance, physical assistance, management of assaultive behavior.
Risk/Safety Conditions: Will have on-going interaction with Individuals. Potential exposure of communicable diseases, blood-borne pathogens, medicinal preparations, and other conditions common to this work environment. May require some exposure to communicable diseases or bodily fluids. Essential physical activities: To include walking, sitting, hearing, and seeing up close, talking, standing, finger movement, repetitive motions. Is required to lift a maximum of 25 lbs. and will ask for assistance as needed lifting over 25 lbs. May be required to repetitively lift unassisted transfers of Individuals with adaptive equipment. Pushing and /pulling one wheelchair/gurney with up to 250-pound Individual.
Maintains sufficient strength, agility, endurance and sensory ability to perform the duties contained in this job description.
