How you’ll make an impact in this roleGuide patients and families through the entire continuum of care, serving as a dedicated navigator to ensure a seamless transition into and through the rehabilitation process.
Collaborate as a strategic liaison between medical staff, administration, and insurance providers to streamline communication and advocate for the patient’s treatment plan.
Analyze care outcomes and patient data to drive process improvements that directly enhance the quality of service provided by the clinical team.
Champion health equity by securing essential referrals and follow-up support for uninsured or underinsured patients, ensuring no one is left behind in their recovery journey.
Cultivate a high-performing team environment within the Patient Access Admitting department, occasionally traveling to local acute care facilities to facilitate patient transitions
Licensure / Certification / Registration:
- Licensed Registered Nurse credentialed from the Kansas Board of Nursing required.
- BLS Provider preferred. American Heart Association or American Red Cross accepted.
Education:
- Diploma from an accredited school/college of nursing OR required professional licensure at time of hire.
May require local travel to acute care facilities.
